Alright, buckle up, because we’re diving deep into the battlefield of stress fractures. These pesky cracks in your bones love to mess with your groove, but fear not! We’ve got the ultimate weapons in our arsenal: footwear and custom orthotics.

Cracking the Code of Stress Fractures: Let’s Talk Shop

Stress fractures are like that annoying chip in your windshield that keeps getting bigger. It starts as a small problem. It slowly grows until you cannot ignore it anymore. But guess what? We’re not letting that happen.

Early intervention is key, my friends. We’re talking about nipping these fractures in the bud before they turn into a full-blown bone party.

There are a few things that lead to the perfect storm for stress fractures. They are biomechanics (blame your parents), Anatomy (blame your parents), activity, shoe selection (blame your fashion sense), and increase in body mass (blame gravity).

I am sure your parents gave you plenty of good traits other than poor foot mechanics and deformities. If you have inherited hand-me-downs (Bunions, hammer toes, and/or flat feet) you are on the preferred guest list for the stress fracture party.

For runners: If training exceeds recovery/healing time, you may get the invitation for a stress fracture. If you are holding on to running shoes too long, wear thin-soled or high-heeled shoes your odds just went up.

The majority of stress fractures occur in the metatarsal bones. These are the table legs that hold up the body. The first metatarsal should get the majority of the workload. But if the second metatarsal has to do more work it will get “stressed” out. The second table leg is the most common metatarsal to get a stress fracture. This is a very common secondary problem with Bunion deformities because of the transfer of pressure.

Stress fractures start as a small weakness in the bone. It’s like bending a metal coat hanger repeatedly. It slowly weakens until it fails. The insulting injury is a result of repetitive stress or overuse. They occur in weight-bearing bones of the feet and legs.

There are three weight-bearing bones in the leg and 26 bones in the foot. The Vegas odds are on the foot. Stress fractures can affect athletes, active individuals, as well as bone-compromised people. I anticipate a spike in stress fractures with the increased participation of Pickleball. An acute increase in activity in a population of osteoporotic people is the perfect recipe.

As the fracture develops there will be an increase in swelling and pain. This is the body’s alarm system telling you the bone is becoming unstable. The significance of early intervention cannot be overstated. Prompt recognition and management of stress fractures can significantly decrease recovery time. Early detection may take as little as 2-3 weeks to recover.

A delay in treatment can keep you off your foot for 6-8 weeks or more. Delayed treatment can also lead to a positional change in the bone. This will lead to reoccurring fractures or transfer fractures to another bone.

Now, let’s get real about shoes. Your choice of footwear can make or break your bone game. Ill-fitting shoes, zero support, and lack of cushioning? Nah, not on our watch. We’re all about shoes that have your back. Think biomechanical stability and accommodation for deformities—like bodyguards for your feet.

Choosing the right footwear is paramount in stress fracture management. Shoes can help distribute pressure evenly across the foot, reducing the strain. Shoes that are very flexible, thin-soled, and elevated heels can add to the risk of a stress fracture.

Say hello to your foot’s new best friend: custom orthotics. These bad boys are like your favorite old bathrobe. They hug your unique foot shape, fix biomechanical quirks, and make walking feel like a dream. Who needs a fairy godmother when you’ve got custom orthotics?

Orthotics come in various forms, ranging from over-the-counter inserts to custom-made marvels. Over-the-counter inserts provide a basic level of support and protection. They are suitable for a small amount of correction or temporary use (like buying reading glasses at your pharmacy). They are affordable and readily available. OTC inserts are a convenient option for some individuals with minimal mechanical problems.

If there is considerable pronation or deformity, there may not be adequate correction in an over-the-counter device. Be very cautious of the Über expensive inserts from the Not So Good Foot Store. These are actually off-the-shelf devices that can be found for $50 to $75 at many online stores. The store has great commercials but these devices cost 3-4 times more than a custom device.
