If you’ve found your way to this page, chances are something has been bothering you with your feet and ankles. The good news is that we’re here to help. The most important thing to understand about your feet is that pain is not normal, and it’s our goal to get you back on your feet without pain, regardless of your age or previous condition. To understand what to expect for your first visit with our office, please read through the following new patient information.

Preparing for Your First Visit
Your first visit to our office establishes a vital foundation for our relationship with you. It gives you time to get to know your doctor and to get you better acquainted with all we offer. It gives us a chance to review your medical history and evaluate your podiatric health. We will perform a thorough examination of your feet and explain to you whether or not you will require treatment, what that treatment would be and why.
Your First Time at the Front Desk
Our goal is to make your first visit as smooth and simple as possible by eliminating pre-appointment paperwork and minimize stress at the front desk. We are able to make this process easy by collecting the necessary information with you over the phone. We find that being able to obtain this information prior to your appointment with a simple phone call can make your time in the office quicker and require fewer forms to complete once you arrive.
However, we do also provide these forms online if you’d prefer to fill them out before you arrive rather than connect on the phone. Although we prefer the first option, we’re happy to accommodate whichever method works best for you.
Insurance and Payment Methods Accepted
Our office participates with many major insurance plans, including Aetna, Anthem BC/BS, Cigna, Encore, IU Health, PHCS, Community Pro Health Network, Sagamore, and United HealthCare. If your particular plan is not listed, we are also happy to accept cash, checks, and credit or debit cards (MasterCard, Visa, and Discover).
A quick note on insurance: some insurance providers we do not currently support may list Dr. Leibovitz as a participant. He is a participate with those insurances at Community Health Pavillion East (10122 E 10th St), where he teaches.

There are two great ways to set up an appointment with Jeffrie C. Leibovitz, D.P.M.: by phone or online.
